It's either failing at the database level, or at the IMAP level. Here are a few items: 1.) Make sure auto_create_user is set to TRUE in the main.inc.php file. 2.) Make sure there's a default domain, and it's proper in main.inc.php. 3.) Double check your server settings in main.inc.php. 4.)
Server Name is mail.xyz.com, I am trying with these settings for Imap is at 993 port of mail.xyz.com, where as SMTP is at port 587.

I would like to customize the login page and remove the words "Welcome to RoundCube Webmail" from the title bar (above the username and password) and from the title of the webpage at the top of the screen so that it references our church.
That's not Roundcube, Roundcube is the name of the webmail software they use. They will probably be able to reset the password for you. Roundcube Plugins : Contextmenu, SpamAssassin Prefs, and more...
Well, I tried, but got "Login failed" message. My account works correctly in various mail clients and webmail clients (mail collector services). There were no errors through Roundcube installation. Thanks for your answer, may be I should try to re-install Roundcube...

I want to customize the Roundcube login page and Roundcube webmail user dashboard with a Company logo. Also, Roundcube should redirect to a website home page on logout. The login page and dashboard customization must be made in a way that it will not be disconfigured by Roundcube updates in the future. Roundcube is runing on a CWP Panel.
When configuring Outlook, make sure you select IMAP. The Internet Message Access Protocol (IMAP) allows an e-mail client to access e-mail on a remote mail server.
